Does divorce happen by signing divorce paper?

Q:

My friend was in nikkah of her husband for 3 years. With passage of time they had some difference and a strong fight before rukhsati due to which she started demanding for divorce. Her husband was not in favor of divorce, they decided to sign mutual divorce deed/talaq nama. The exact words on divorce paper was” I divorce (three times) my wife so and so…” . My friend completed her iddah. Later after 5 months, her husband contacted her saying that he wants to reunite, as his intention was never to divorce her and he only signed the paper upon her pressure.
Now they are confused if divorce happened or not. What does signing of talaq nama without the husband’s intention mean? Can they continue their marriage or new nikkah would be required? Please do answer and advise.

The following is according to the Shia sect.
For a divorce to be correct, it has to have the following conditions:
1- You have to have two believing adults as your witness
2- The woman has to be clean of her menstrual period and also shouldn’t be pregnant
3- Proper Arabic words need to be uttered for the act to take place.
